Replacing Ceramic floor tile and re-grouting

I have an entry tile that has cracked and I don't know how to replace it. Also, what do I do when grout starts breaking out between the tiles? Can I patch or do I remove that part and re-grout? Any info would be welcome. Thanks!

In regard to the grout breaking out. I would suggest removing the grout getting the area clean and then regrouting. There is a new product on the market that is acid/harsh chemical free that will soften/granularize the grout so removing it doesn't damage the tile (when the grout saw slips). It also doesn't etch the surface of the tile. You can find it at www.betterbiosolutions.com.

wondering if the acid/harch chemical free would work or is there other steps to the process? Any suggestions would be helpful. Thanks a bunch. maricee4fun
